Can somebody explain the end of the episode of Sherlock entitled A Scandal in Belgravia? What happened to Irene?

(After the meeting of John and Mycroft at the end. The beheading part.)

Sherlock episode entitled A Scandal in Belgravia ends at the point where Sherlock infiltrates the cell and rescues Adler with only seconds to spare.
