The cockroach will not die if it manages to get back on its feet after being turned on its back. However, if it was sprayed with an insecticide before turning on its back, then it will eventually die.

Okay... No. I helped a little cute beetle by letting it hold on the stick and it got back up and left. I would kill it if I were you, because it can cause infestation in your house.
