How to write a request for employment change of status

i am currently wanting to go down to on call status at my job but i dont know the polite way to phrase it and i want to do it the correct way with a 2 week notice

I would very much appreciate your written decision and confirmation of my request, at your earliest convenience.

Again, thank you for your consideration in this matter. If you have any questions, please feel free to contact me at ________ anytime.
