It first was a name used for head lice. Children get it the most, as they're in close contact with other kids. So it became a girl & boy joke, don't get close to him/ her they've got cooties, thankfully we always had cootie spray on hand to destroy cooties

They are bacteria and germs that "girls" have, they really don't have cooties they are not real. Boys just say that to girls because they are girly or something, but usually girls get called like that when they are younger. Trust me when I was 6 years old boys told me I had cooties, those were the young days.
