I use to have a book it was a choose your own adventure book that had a character sheet.... I just can't remember the name.

I need the name of the book and the series, it was about a sorcerer that was fighting some kind of evil.... I know vague, but it was a long time ago when i had this book.

I think that the "Choose Your own Adventure" book you are referring to is called the Zork books. This is a series of a total of four books about stories that took place on the fictional universe of Zork. Check out http://en.wikipedia.org/wiki/Zork_books for more details on the books.
