Why is it in duval county you can be in poverty and still get turned down for medicaid

My brother in law had this problem in Texas, He had been given as a preacher the option of not paying into Social Security so when he got desperately sick SSI would not help him. He spent many weeks trying to get on medicaid before he was finally allowed on, my advise is keep trying they will eventually give in.
