What does it mean when a teacher puts "demonstrates best effort" on your report card?

I have an A in my 5th period class, and my teacher put this.I also got "O"'s (outstanding) in work habits + citizenship. Does it mean...?

It means that out if all the students in your class, you were the one that stood out the most as the hardest worker and that you took pride in your work. It means that your teacher would have liked to have a whole class of students that behaved as you did and who worked as hard as you did. It means that when she was adding up everyone's grades, when she saw your name, she smiled because you were respectful of her and her class and of yourself for wanting to learn . It means she wanted you to know that she had been watching you work hard this year and she wanted you to know she was proud if you, so she gave you the extra good grades she did. It also means that she thinks you will make a great 6th grade student !!!

Well that is kind of unconventional as the grading system usually goes A, B, C, D, F. I suppose O would mean what it means - outstanding. Nice work!

It means you are a good student. You did your work well, (hence the A) and you work hard and are not disruptive in class. (Outstanding in work habits and citizenship)

The demonstrates best effort to means you couldn't have worked any harder than you did.
