I make my hot chocolate with real chocolate--the best I can get my hands on, usually ghirardelli. Place a couple ounces of the chocolate in a small saucepan and add 8 ounces of milk (or, for even more indulgent, add some half-and-half cream). Heat stirring constantly, until the chocolate is melted and the beverage is heated to your taste. At this point, taste it--you may wish to add a little sugar. Remove from the heat and add 1/4 tsp of vanilla extract. You can jazz it up with ground cinnamon, peppermint, orange peel (NOT orange juice!), or liqueurs of choice. Another way to add unusual, delicious flavors is to add flavored coffee creamers--though I prefer the type of creamers made with real cream.
