Can a company that runs a factory with 3000+ production workers require that the worker to ask/receive permission before using bathroom

Permission? .. not exactly .. That is a basic human need. I believe, pretty much everywhere these days (in North America) .. They are to provide you basic human needs ... But .. if you are performing a task that requires a replacement to take over your position while you take a potty break in an 'emergency' .. that is a different matter. They need to provide you your replacement for your break in a reasonable amount of time.

I guess it would depend on company policy. Sounds more like a kindergarten. A grownup should not have to ask for permission.

I do not think that the workers are required to ask for permission, but also this will greatly depend with the company's policy. I would suggest that you first consult with your supervisor or manager for more help.
