Who do I go to, to get a form for super/ street for a number issued to me ?

You can go to any NHRA offices next to you. They will have all the forms you need to fill out for the numbers you need. You can also go online and download the forms. Have a look at http://www.nhra.com/competition/forms.aspx for more information.
