Mutation is not necessary by chance. You can make something mutated. For instance you could consume lots of drugs and alcohol while pregnant, or you could raise something in a pond full of oil, or you could mutate your own body like a guy who cut his tongue in half.

The only option would be crossing over (if in the religious form) because that doesn't happen at all.

Like crossing over in the fact that it occurs at the stage when chromatids of homologous chromosomes pair up during synapsis, forming X-structure (chiasma). The chromatids break into segments (of matching regions), which are then exchanged with one another.
