How do I remove deep blackheads? Underneath skin?

You could try to use steam to open your pores then try to work them out. Heat up some water then put your face over the pot and cover your head with a towel to trap the steam. Make sure the steam is not to hot. Hold your hand over it to test it. You could also get a facial that targets deep extractions.

Take a hot wet wash clothe and let it lay on your face for several minutes. Then apply hydrogen peroxide. This will clean and loosen the area then try exfoliating. Try not to pinch it out, that will only spread the bacteria.
