Do you mean a deductible? If so, then normally yes. You pay out of pocket until your deductible is met and then after that your insurance only pays so much and rest of the cost is still left up to you. It usually just depends on the type of insurance.
I think there are instances when you have to pay out of your pocket for medical support in case you get private health care for your children. But if you had insurance policy for your children, then you will have to send a claim form to the insurance company for compensation.
