What's making small holes in my Papayas, maggots in some, prematurely dropping off tree, and how do I prevent this?

The trees are healthy but have not been able to harvest an edible fruit, 20 to 30 wasted due to above problems!

Web worm, earthworms and hornworms are known make small holes in papayas and maggots. They can be prevented by spraying the plant with a permethrin or malathion before the plant starts to produce fruit.

buy a pesticide. Look on the under leafs for insects/caterpillars. It might just be a bad tree that happens more often than people think. Where is it planted?

I dont see any or evidence of any insects. I have 3 trees, 2 producing fruit and the third has blooms on it. They are in my backyard, full sun, with mulch around them. Could it be birds poking holes? The holes are about the size of the point of an ice pick
