Going back to my home town...

And I am visiting a friend that I haven't seen in about three to four years. We have stayed in contact by email, and phone, and Facebook, but how to I act around her without making it awkward?

It should be natural to get back to where you were, especially since you have been in touch. Tell her about your trip, ask her what's been going on... You will be fine.
