Probably both mouth and nose. So if you "Want to Win Friends and Influence People" you may want to get your little problem taken care of, because even if don't sneeze on them directly, they may not appreciate the sight of blood on your handkerchief. And if they know you just came out of an HIV intensive care unit, you could die of something other than a virus. Just saying.

Actually if you have an active bleeding nose sometimes it is a blood clot that prevents your nose from stopping bleeding. I realize it's a paradox, but often times in the ER we will try direct pressure first if that doesnt help we will have the patient blow their nose gently and re apply direct pressure. Don't be afraid to sneeze but be advised there could be blood spray. SO move away from anybody or anything you dont want to risk getting blood on. Make sure you cover your mouth and nose with a good amount of tissue, dont block your mouth or nose just cover it to prevent spray, and sneeze normally. Afterwards wipe, and then try a gently blow of your nose, again using generous amounts of tissue to prevent spray. Then reapply pressure. That actually usually does the trick. Goog luck.

When yo sneeze and it is bloody, it is usually due to dry linings on the inside of the nose (nasal passages). It will not hurt you. If you have constant dryness inside your nose, you can use a saline solution (spray) or a neti pot.

If you have a bloody nose (as in blood flowing freely from your nose) you will not want to sneeze anyway. The nasal passages will be moist with blood and will not trigger the sneezing reaction.
