To be an ironman triathelete world champion would I have to quit my job or can I train on the side?

Like, could I be a doctor and train for triathlons on the side and win?

Depends on the type of doctor. Some doctors put in too long of a day to train that hard. You might be able to train to be well enough to get a sponsor and then not need to work while you train to win.

That would need a lot of time, effort and training, so it would take a very long time to train on the side.
