I'll assume it's your first one, otherwise you'd know... Nah, it doesn't hurt, but if you're very really worried & tense, it might be a little uncomfortable & it is kind of awkward. It's really important to have the test done, the doctor is checking for cancer cells on your cervix. Cervical cancer has virtually no symptoms until it's in late stage, so going for that yearly check-up is the best way to diagnose & treat it early. So... don't worry, it'll be over before you know it! :)
They always tell you to relax as they check you. Now having had tons of these exams in my 60 years, I will tell you that, that is easier said then done. The truth is its always best to keep your muscles as relaxed as possible. I wouldn't call it painful. It most certainly is very uncomfortable. My daughter has her first Pap smear this year and she's not looking forward to it.
