ive had that happen, even way before the latest software. my mother has had it happen on her crappy little LG phone too. idk what it is but i normally would just ignore it and my phone would charge anyway.. or i'd unplug it and plug it back in. if its preventing you from charging it completely tho i'd contact apple

Do a back up to iCloud then do a restore with in the phone or with iTunes do it first as a new phone and try it if it works do a restore back to ur cloud back up and yet it. Could have been a simple line of code error that happen.

The software probably isn't agreeing with the hardware in your iPhone 4. This tends to happen on older models of the iPhone. I suggest that you get a back up the phone, wipe it, and restore your content or get a newer iPhone... Or a Samsung.
