What causes bad breath that doesn't go away with normal dental care, brushing your teeth, flossing and mouthwash?

Anybody on Ask.com ever had this issue and if so, what caused it?

Baking soda absorbs odors. I add a teensy bit in with my toothpaste, and it usually works.

It's usually gingivitis. Do your gums bleed when you brush??
