Did you feed her something or is there any possibility of her eating something she usually doesn't? That could be it. If she is still sick after a day or two take her to a vet, it could be something more serious.

Theres two possible things that could have hapended

1.) The diarhea could have been somtething just normal just like humans would get diarreah. The darrhea could have also been from something that she ate...for example you could have dropped food next to her cage that she ate and made her sick. You may have rubbed her too hard and that caused a blood vessel to pop or you may have hurt one of her vital organs.
2.) She may have just gotten sick

I'm so sorry you lost your sweet friend!
I really doubt it was anything you did wrong!! You kept her healthy and loved for 2 years, so you did good!
Rabbits rarely get diarrhea because the way they digest their food is different from dogs and cats. When they do get diarrhea, it is usually a sign they are seriously ill. It's impossible to say what was wrong (maybe a rabbit-experienced vet could do an autopsy or tests, but I wouldn't bother since that's pretty expensive and the poor thing is gone already) --
But I seriously doubt it was anything you did wrong!!! Do not blame yourself.
One of the most common causes would be an intestinal parasite that she'd had since her shelter days, or was even born with. These often kill babies in infancy but your girl made it this far... And if so, it was because you took good care of her.
It could have been an intestinal problem compounded by imbalanced diet (their inner workings are complicated so I will not expand on this) -- but, again, that would have shown up earlier in 2 years.
If, at some future point, you get another bunny, make sure it's checked out by a rabbit-experienced vet (not all are). He/she can help you with optimum diet tips and do routine wellness checks.
Basically, I'm so sorry for your loss, but sometimes these things just happen. You did the best you could!!
