Low self esteem, fear of change. Lack of resources. Family. Time. Apathy. Overall, a negative outlook on ones abilities. Don't allow these things to creep up on you, and you could amaze yourself on your potentials!
I would venture to say, most often we ourselves hold ourselves back. by our lack of faith or our being determined enough or our impatience and a host of other things. but we have it in us, to accomplish pretty much anything we truly want! and to never, never, never, NEVER give up...;-)

Failing to reach ones full potential can be caused by allowing distractions to defeat one's focus, indecision, fear, pure misfortune such as injury or disability, and more often than not, laziness. Laziness encompasses everything from failing to seize opportunity to refusing to seek it.
In order to reach your potential, you always have to do the best you can. To many settle for just passing, because being the best you can be requires total commitment. Our species is big on taking the easy way.

One of America's top chess grandmasters and teachers, Yasser Seirawan, once said that chess players tend to fall into two categories: those who think too highly of themselves, and those who don't think highly enough. Of the two, he said, the second is the worst.
I think lack of confidence coupled with lack of determination is the biggest stumbling block we have to get over. If you believe in yourself you can accomplish much.
Inadequate pre-natal nutrition and maternal care Lack of access to education and health care. Physical, emotional and/or sexual abuse. Drug/alcohol abuse (esp. fetal alcohol abuse) Brain injuries. Etc.
A lot of the time it's fear of man. People are worried about what others think. The Bible has great advice, though. Proverbs 29:25 tells us "Trembling at men is what lays a snare, but he that is trusting in Jehovah will be protected."
