To add a parent to the birth certificate as the parent, Dow that parent need to be present?

Report as

You'll be creating a new birth certificate, the parent to be added will be required to sign on the new one. This can be done through the City Clerks Dept at the local courthouse.

Ok thank you.. And so you know if they will be able to get my sons social because I don't want him to

I don't recall the child's social going on the new birth certificate, if that's what you mean. Is the 'him' also the baby's daddy? Because he will have to sign the new one, then you can go after him for child support.
If you ever need assistance the father listed on the birth cert will be expected to pay child support, no way around that, I'm afraid. But no, your son's social won't be revealed. Anything else I can help you with?

Yes. They must sign a (I don't recall the name of the document, exactly) to verify that they are the parent.
