What kind of things should I write in my weight loss/food/goal journal?

using the journal until I reach my body transformation goal

Exercises you did, a score of 1 to 10 of how much you would want to do that exercise again, what you ate, what minerals and vitamins you consumed throughout the day and what percentage of the recommended you consumed, what times you ate at so you can stay more constant every day with what times you eat at, your weekly weight....

like how much u weighed in the beginning, what food u had ate and the calories(if you are counting calories), your goals i guess and whenever you checked your weight
