He was shot onstage while performing with Damageplan at the Alrosa Villa in Columbus, Ohio. The gunman, Nathan Gale, shot Abbott three times in the head using a 9mm Beretta 92FS handgun; the third shot killing him. Gale continued shooting, killing three others and wounding a further seven. Gale fired a total of fifteen shots, stopping to reload once and remaining silent throughout the shooting.
Early theories of motive suggested that Gale might have turned to violence in response to the breakup of Pantera, or the public dispute between Abbott and Pantera singer Phil Anselmo, but these were later ruled out by investigators. Another theory was that Gale believed Abbott had stolen a song that he had written. Gale was severely schizophrenic and had stopped taking his medication.
Abbot was shot and killed while on stage during a Damageplan performance on December 8, 2004, at the Alrosa Villa in Columbus, Ohio. He ranked 92 in Rolling Stone magazine's 100 Greatest Guitarists and #1 in the UK magazine Metal Hammer.

Did they ever give his name? I can try searching, but I don't know how much info there is on it. Apparently he was just some psycho who claimed that they stole his lyrics. He was just paranoid, or had something wrong with him mentally (or he was just a total jerk who deserves to suffer) Don't worry though, the douche bag didn't go without punishment. R.I.P. Dimbag :'(
