Is there a specific condition or psychological disorder that would cause an elderly permed to suddenly be very mean and say hurtful things?

Could be the onset of dementia. Alzheimer's, especially will manifest itself in personality changes. If they are aware of changes going on they can become very defensive or offensive.
I have over 20 years of experience in elder care. Have a complete workup done; have them check specifically for dehydration and an Urinary Tract Infection (which sometimes mimics dementia) as well. Contact the Alzheimer's Association to learn more about the disease and how to cope with these personality changes.

As mentioned, have her checked for a UTI. This mimics dementia, and at this age, it could be dementia, OR Alzheimers. If it is alzheimers, or dementia I recommend a book titled: "A 25 hour day" It will be good for your whole family. This is a tough road.
