How do i give my sons godmother rights

i am a single mom when i was at the doctors office they had mentioned something about a form that states that for some reason i cant be reached or something happens to me i could name someone to make all the medical decision for my son does anyone one know what this form is called or any ideas about this kinda of stuff again i just want that if an emergency happens at daycare or school or something and for some reason i cant be reached i have a backup person i dont want to give this person guardianship all the time

You could write a letter stating in the event that I can't be reached so-and-so has permission to make all medical decisions for my son. You should also date this, for let's say 6 months at a time, and you would have to redo it every 6 month. I truly don't know what form you could use for that.
You should also do a Living Will. Or Duel Power Of Attorney. That would be where you have yourself covered and your son. Your son's godmother could make all medical decisions for him, if you are unable to make the decisions yourself. She could also make them for you, if you are unable to make them. I hope I helped.

A godmother is usually a close friend relative.
It is your choice, to who you choose and trust to take care of your child if something ever does happen.

but you this sounds to me Like an emergency contact.
meaning again just choose a friend or relative you feel comfortable with watching your child.

There is such a thing as a Medical Power of Attorney, which might be what you are talking about. To clarify if this is the form you are talking about, you should talk to a doctor or an attorney about it; they will direct you on what steps you need to take and the consequences of such steps.
