You should consider giving your friend a long time present. You can write long letter of how much he/she means to you, and then frame it. This will last him/her more that a life time; he/she might even keep it to remind him of his birthday.

This person is your "really good friend" and you know him so well that you want others to pick the birthday present you should give him? OK, if you are his friend, feed his fantasy.
Every male has his 'secret' (Read: "known to friends") wishes. Might be as simple as always wanted a pair of bright red swim trunks or never thought he should spend the money for his very own purple bowling glove. Could be as expensive as he always hated shoveling snow but is too thrifty to buy a snow blower or really would love to have a bass-fishing boat.
Bottom line - Cowboy up on his one, aim at prices that are above your base comfort zone and get your really good friend what you darn well should know he wants and needs.
Go to a location that is very special to him such as a park, spot on campus, marquee of a movie house, etc. Then photograph it, and put in a nice frame with a mat. If he is a student, have it made into a poster instead.
You could get him/her a gag gift like if they love bacon you could get them bacon lip balm or soap. A invaders Zim fan could have get a zip up hoodie or, whatever they like such as their favorite bands' new album etc.
Ribbon embroidery or cross-stitch. They are difficult, but if you hold to it until they are finished, ,you will see what an amazing job you've done! I've embroidered a wallet with BBF's name on it. Her name was lily, so I chose one with lily flower. She couldn't tear herself away from it from the moment I surprised her. And she always showed off her new purse in front of her friends. She told me that she just enjoyed the jealousy of people with whose eyes fixed on her purely hand-made boutique. And I'm glad that she loves it so much. I know it is not what kind of gift you give them, it is with what kind of intention that matters. They just love whatever you made with your whole heart in it. If he is a good friend you should have a rough idea of what he likes, think about what he likes, and be creative! I am experienced at sewing and love experimenting with different fabrics, embroidery and just nice arty type things, so try it! Homemade things are valued very much
If your friend is a girl, get her some awesome jewelery , I found a cute necklace with a doughnut on it! It was so cute! If your friend is a guy, buy him a skateboard, I mean what kind of boy doesn't like skateboarding? (ok maybe some don't like skateboarding )
How about a Japanese Bonsai Tree "with instructions". No two are ever alike, they are petite, beautiful, unusual. It will change as you both will, and just like life she, can help shape it's future along with her on. But mostly, no matter where your life's take you she will smile, think of you ever time she walks by or is just chillin" in her room, and all the great times you had...and then your phone will, you really can't say ring anymore can you, but you'll be chatting away. Hope she likes it! Oh, just call your local nursery, ask for their expert on Bonsai trees and take it from there. Bye Bye
Get him whatever u can afford, a good friend will appreciate anything u get them, but still keep in mind if it's good enough fr u it's good enough for him, don't try to go overboard and end up with financial problems.
Perhaps an expensive gift he would like. Money seems good but probably will end up as a regret in your later years as he might ask for more. Get him what he want's and remind him that your always there for him. :)
It all depends on personality, if they're a happy outgoing person, go big! But if they're a more proper, containable person, maybe you should go a bit small. It also depends, are they're your friend, good friend, or FREAKING BEST FRIEND! Go on what you think he'll like! If they're really your good friend, they won't care if it's not totally perfect and amazing, they'll settle for a great gift and put whip cream on your hand whilst you sleep. Do the exact opposite of what I said if they're more proper. They'll want a more humble and amazing gift. (I think)
It depends on what he/she really likes. If you know what your friend's passionate about then get them something that contributes to that. I am a Hello Kitty fan and my good friends give me Hello Kitty gifts sometimes because they know that I like it.
