A doctor in a private practice can refuse to treat anyone they choose. And hospitals often choose to not treat people who dont have insurance. My personal experience with this was with a chiropracter. Id hurt my back and was supposed to have three visits to a chiropracter. First session was awesome. They were very attentive and the chiropracter himself was very friendly and did all sorts of cool stuff. On the way out i paid in cash. They did file on my insurace tho and found out i wasnt covered for chiropractic visits. Despite the fact i paid in full in cash the second visit left much to be desired. I was keft in the emoty waiting for thirty minutes, saw the doctor for about one minute during which he did nothing and didnt smile once and seemed in a hurry. The assistant did one procedure and was finished. I paid in cash again. Did not go back for the third visit.
