A Living Will is a form you fill out which is usually notarized by a notary or attorney, stating your wishes at the end of your life. You indicate whether you wish to be kept alive with tube-feeding, oxygen, and so forth to extend your life. You can opt for "DNR"; Do Not Recesitate" for no extraordinary means taken. This way your survivors will have your decisions plainly in front of them and know that you have made your own choices. You can speak further about a living will with your attorney or doctor.
