The only way it cannot be mono or strep type A is when you performed the actual test. In the case of mono it isn't necessary and may be expensive and does not contribute to management unless for epidemiological research purpose.
In the case of strep A it is not routinely done as the treatment is simple cheap and empirical. That is one of the reasons why rheumatic fever and heart disease is now rare; "Empirical Treatment" Again only useful in suspected outbreaks and for research purpose.
As for what else it could be; the answer is "search me" :(
