When would a chain of custody from be used? why would this form be needed?

It is used to trace lineage of a sample from time of collection to result reporting and sample disposal. The original copy of the form is given back to the client with analytical report and project copy file is retained.

Also used for evidence used in a crime. It will get collected, bagged & tagged and signed. Then signed by whoever else it comes in contact with.
