Why is it important for nurses to sign each entry when documenting?

Signatures on documentation are very important to establishing standards of accuracy, authenticity, and traceability. The signatures for each entry allow others to audit the document and process, trace the sequence of event, and evaluate performance.

because of legal issues that might occur during your shift that they know which nurse has taken care of that patient
