Is all medical tourism wrong? Are the patients or the local people being exploited?

No, not all medical tourism is wrong. It is good because healthcare providers are able to travel to other regions to deliver healthcare which may very well be needed and this can save a lot of lives. It also serves as a national industry and the patients are not exploited.
