Medical assistants do not treat or deal with direct medical diagnosis. They are extremely limited in what they do and it's usually vital signs, phlebotomy, EKG, and minor things like throat cultures.

An internist, in its name, has nothing to do with the liver. Any type of doctor can be an internist. They are physicians in training who have completed medical school, has a medical degree, but is not allowed to practise medicine unsupervised. A doctor of the liver is called a hepatologist. A hepatologist can be an intern, just as a cardiologist (heart doctor) can be an intern, a nephrologist (kidney doctor) can be an intern, and so on.
