You can apply for Child Health Plus coverage though New York State of Health Marketplace. Only online applications are considered. The eligibility criteria need to be verified before applying for Child Health Plus.

Children who are under 19 years of age and residents of the state of New York are eligible for either Children’s Medicaid or Child Health Plus. If the family income is greater than the levels given by Children’s Medicaid, then you can apply directly through a Child Health Plus insurer.

There is no need to pay any monthly premium for those families whose family income is less than 1.6 times the poverty level. This works out to about $600 per week for a three-member family and $724 for a four-member family. Families with higher incomes have to pay monthly premiums of $9, $15, $45 or $60 per each child per month, depending on the size of the family and their income. If the family income is greater than four times the poverty level, then they have to pay the full monthly premium.
