What is an "in case of emergency" form?

Answer

These days just about everyone has to fill out a form stating who should be contacted in case of an emergency. You need to fill this out at the doctors office, the school, your job, and anywhere you register or fill out some kind of application. The ICE program was created as a way to help first responders and hospital personnel know who to call if something where to happen to you while you were at one of these places.
